Edberts House is a thriving community charity currently managing four community projects based in Gateshead. We work together with residents of all ages to make our communities happier, healthier, and more welcoming places to live.
Over the past five years our Community Linking Project has contributed to the health and wellbeing of patients in Gateshead by linking them to a range of community services. We now have an exciting opportunity for Community Link Operational Leads to join our social prescribing team. You will line manage a small team of Community Link Workers, based in GP surgeries within a PCN area, developing one-to-one and group support networks, meeting regularly to discuss cases. You will be responsible for team training and development, working together with the Development Manager on recruitment and areas of project development. You will also be responsible for managing a case load and coordinating cover for Community Link Workers when necessary, supporting patients who have visited the GP and/or partnership organisations by helping them explore what is important to them In a person-centred way and accompanying individuals to local, non-medical sources of support.
Candidates will come from a community, information/advice, social care, or health background, must be excellent communicators and able to work in a multi-disciplinary team. You will need an interest in working with people with multiple and complex needs, and experience of supporting people with long term conditions would be an asset.
Hours are 37 per week, Monday to Friday (some weekend & evening flexibility required). Part-time and job share will be considered.
An enhanced DBS is required for this post.
