Edberts House is a thriving community charity currently managing four community projects based in Gateshead. We work together with residents of all ages to make our communities happier, healthier, and more welcoming places to live.

Over the past five years our Community Linking Project has contributed to the health and wellbeing of patients in Gateshead by linking them to a range of community services. We now have an exciting opportunity for Community Link Workers to join our social prescribing team. Working across a Gateshead Primary Care Network (PCN) you will support patients who have visited the GP by providing advocacy, signposting, and accompanying individuals to local, non-medical sources of help. You will proactively build relationships with patients and community organisations to support individuals to come up with innovative solutions to problems.

Hours are 37 per week, Monday to Friday (some weekend & evening flexibility required). Part-time and job share will be considered and there are permanent and temporary roles available.

Candidates will come from a community, information/advice, social care, or health background, must be excellent communicators and able to work in a multi-disciplinary team. You will need an interest in working with people with multiple and complex needs, and experience of supporting people with long term conditions would be an asset.

An enhanced DBS is required for this post.
